ILL has quickly become one of the most talked-about horror projects after Sony’s latest State of Play showcase, and the reason is hard to miss: the new trailer leans heavily into extreme violence. The footage has already pushed many viewers to call it one of the most disturbing horror games shown on a major stage.
The first story trailer did not soften that impression. Insider Gaming went as far as saying the game “could be the most violent game ever made” after seeing scenes filled with blood, gore, and unmistakably brutal imagery.
A survival horror setup wrapped in mystery
ILL is being developed by Team Clout as a first-person survival horror game. The player wakes up inside a research facility, where the character appears to be held captive by a mysterious figure.
What is happening inside that location is still kept vague, but the environment shown in the trailer already makes the danger clear. The facility is filled with grotesque creatures and disturbing experiments, creating a setting that feels hostile from the start.
Brutality that is hard to ignore
The trailer’s most memorable moments center on shattered skulls, decapitations, and heavy bloodshed. Because the graphics are detailed and the physics look convincing, the violence comes across less like exaggerated splatter and more like something closer to a punishing horror film.
That approach gives ILL a different identity from many other horror games that rely more on subtle tension. Instead of quiet dread, the game appears to favor raw, direct shocks that are meant to hit immediately.
A familiar format with a much harsher edge
The gameplay presentation points to a classic first-person survival horror structure. Some viewers have even compared it to Resident Evil 7, although ILL seems to push gore much further than that familiar reference point.
At the same time, the game’s tone places it far away from horror experiences that build fear slowly. Silent Hill often works through subtle unease, while ILL appears to choose more intense and explicit violence as its main source of terror.
Strong reactions from the audience
The reaction online was immediate, especially in the YouTube comments under the trailer. Many fans treated ILL as one of the biggest highlights of the State of Play presentation.
Some were also relieved to see that an earlier concept had finally turned into a real game. Even so, others urged caution and said enthusiasm should stay grounded until the project is actually released.
One of the standout horror games on the horizon
With what has already been shown, ILL is now being watched as one of the most intriguing horror releases expected in 2027. Its blend of extreme visuals, dark atmosphere, and first-person survival horror design makes it stand out clearly among Sony’s announcements.
State of Play did include other major reveals, including a new God of War game. Even so, ILL ended up being one of the trailers people kept talking about the longest because of the brutal impression it left behind.
